Presenters will share real-world models from FQHCs, rural hospitals, and urban health systems that integrate clinical workflows, remote monitoring, digital engagement, and team-based care to address racial disparities and improve outcomes for pregnant and postpartum people, particularly in underserved populations.
Attendees will learn how a comprehensive diabetes care model was implemented in a resource-limited setting, how personalized text messaging and remote monitoring are reducing hypertension-related maternal mortality, and how primary care practices are incorporating universal preconception care into existing workflows to reduce risks before pregnancy.
Together, these approaches offer practical strategies for integrating digital tools, improving care coordination, and scaling prevention efforts to advance maternal health equity.
